Care Partners

Your support person—such as a family member, friend, or caregiver—is someone you trust who can help you in many ways throughout your care journey.

A support person can:

  • Help you remember important information from your appointments and ask questions you might not think of.
  • Offer emotional support and encouragement during stressful times.
  • Assist with practical tasks, such as organizing transportation, keeping track of medications, or helping with paperwork.
  • Be an extra set of eyes and ears to help you understand your care plan and make informed decisions.
