Overview
We support people in Eastern Ontario who survived childhood cancer.
The Pediatric Oncology Group of Ontario (POGO) Aftercare Clinic at The Ottawa Hospital supports people in Eastern Ontario who survived childhood cancer. Our team works with CHEO to help you move from pediatric care to adult survivorship care.
We focus on monitoring the long-term effects of your treatment, providing emotional and social support and creating a follow-up plan that meets your needs.
Team
Our team includes a hematologist, radiation oncologists, a general practitioner in oncology (GPO), a nurse practitioner, a registered nurse, a social worker, a school and work transitions counsellor, clerical staff and Clinical leaders .
What to expect
Referral process
Your referral from CHEO or another centre is handled through New Patient Registration (NPR) or by the POGO nurse. Once we receive it, your information is entered into Epic, our electronic health record system.
Transition document
CHEO prepares a transition document that summarizes your care history. This document is either scanned into Epic or shared with us electronically via EpicCare Link.
Clinic visits
You’ll have an initial consultation, followed by regular follow-up appointments. Depending on your needs, these visits may include assessments by nurses, physicians, and other specialists such as social workers or school and work transitions counsellors.
Pre-clinic calls
A nurse may call you before your appointment to review your needs and ensure everything is ready for your visit.
Needs assessment
Before your appointment, you will receive a questionnaire through MyChart. This helps us understand your needs and plan your care. Learn more about MyChart.
Additional support
- Psychosocial services: CHEO continues to provide emotional and social support during your transition. At our clinic, a social worker and a school and work transitions counsellor are available to help you.
- AYA cancer care enhancements: The Ottawa Hospital Cancer Centre has an Adolescent and Young Adult Program. Services and resources from this program are available to support you.
Referrals
All patients must be referred to the POGO Aftercare Clinic by a physician.
