Immunization
Immunization is an important program offered by our clinic to babies, children, adolescents, and adults of all ages.
Mental health
We have a shared mental health team that supports patients at both AFHT clinic locations.
Diabetes Education Program
The Diabetes Education Program is available to patients who are rostered with the Family Health Team that are at risk of developing or have type 2 diabetes or prediabetes, and are over the age of 18.
Nutrition
The Ottawa Hospital Academic Family Health Team has registered dietitians (RDs) who use their expertise to provide team-based care for you and your family.
Maternity care
We provide pregnancy care for AFHT patients, as well as for patients who get their care elsewhere.
