Academic Family Health Team

The Academic Family Health Team is a teaching clinic of The Ottawa Hospital. 

We provide primary health-care services delivered by a diverse group of professionals, including doctors, nurses, social workers, dietitians and psychologists


We have been experiencing a high number of calls from patients and families about cough and colds.

We recommend you consult this information on respiratory illnesses from Ottawa Public Health first. It may help answer your questions about common cough and cold symptoms and how best to look after yourself. ​​​​​If after reviewing this information you have ongoing concerns, please don’t hesitate to call our clinic.

Appointments

We request that you call first to make an appointment for most issues. We only service patients registered to our clinic who have scheduled appointments.  

Our clinics are not accepting new patients. If you are looking for a family health care provider, you can register with Health Care Connect, a provincial program that helps connect people without a family physician to providers who may be accepting new patients. Register by calling 1‑800‑445‑1822 or by visiting the Health Care Connect website.

If you need to cancel an appointment, please notify reception 24 hours beforehand. Other patients will use your appointment time if you cannot use it. 

Our phones are answered between 8:30 and 11 a.m., and between 1 and 3 p.m.

Frequently asked questions

The Academic Family Health Team is not a referral service or a walk-in clinic. Only patients who have their family doctor at one of our clinics at The Ottawa Hospital are seen. Unfortunately, we are currently not taking new patients.

If you do not currently have a family doctor in Ottawa, Health Care Connect may be able to help. Health Care Connect helps people from Ontario who do not have a family health-care provider (family doctor or nurse practitioner) to find one.

If you are covered by OHIP and currently do not have a family doctor, you are still able to receive recommended cancer screening through the Champlain Screening Outreach Program

MyChart is a secure, online patient portal that makes it easy for you to access your health information from anywhere. It is also a fast and easy way for the Family Health Team to send you letters and important messages. To sign up and for more details, please visit the Your Guide to MyChart page

If you have prescription medications that need to be refilled, please try to book an appointment before you run out. Your provider will have given you enough medication until the next time you are expected to be seen.

Prescription renewals without an appointment are not covered by OHIP. To ask for a prescription renewal without a visit, please ask your pharmacy to fax the requested medication to our office at 613-761-4200.

Your provider will review the request and may refill your prescription for a fee (usually 20$), or they may ask our front desk to contact you for an appointment. Please note it can take 5 business days to respond to faxes for prescriptions.

Pharmacists are also able to renew prescriptions for chronic medications that no longer have refills to help ensure you have sufficient medication until you can be seen for a follow-up.

Tips for managing your prescriptions

  • Make an appointment to come see us before your medication is finished. Make sure to call at least 2-3 weeks in advance.
  • Make sure that you have enough medication to last you until your next visit.
  • Write down the date of your last refill on the calendar. E.g. if you run out in July, make sure to call to book an appointment to refill in June.
  • Bring all of your medications (no matter where they are obtained) to each appointment in order to ensure accurate refills and to keep the clinic medication list up to date. 

When it is necessary, patients are referred to other medical specialists who are not a part of our team. Your family doctor will determine if you need to see a specialist and will make the arrangements.  

If you feel you may need a referral, an appointment with your doctor is required to discuss this. If it is determined that a referral is indicated, your doctor will arrange for this.

Many common services do not require a referral. These include massage therapy, physiotherapy, and orthotics/chiropody care. If you require a letter for your insurer to cover these services, please contact the clinic. As this is an uninsured service (i.e. not covered by OHIP), there is a fee associated with this

Expired health cards cannot be accepted. You can renew your OHIP card in person at ServiceOntario locations or it can be done online. For details see the ServiceOntario website

If you live in the province of Quebec, or do not have provincial health coverage (OHIP), all clinical visits are considered fee for service and are to be paid at your appointment. Payments can be made by debit, VISA, Mastercard, or cheque.

If you have a question about your invoice, please call our main clinic line, and select the telephone menu option for “billing inquiries.”

​Once payments have been received in full, a receipt will be issued along with an Out of Province claim form. Quebec residents can send those to the Régie for reimbursement. The time limit to claim these expenses is 3 months from the date of your appointment.

If you are a patient without any Canadian health coverage there may be a fee charged by The Ottawa Hospital, in addition to the medical fee for the services listed below. Not all our services are covered by OHIP. You may be asked to pay for these services at the time of your visit. Please consult our list of services and fees for more information.
