A referral is required to be assessed and to enter our inpatient treatment programs.
For patients currently admitted to a hospital
Referrals should be sent using the Ocean eReferral system using the listing “The Ottawa Hospital - Inpatient Sub Acute Care TOH Rehabilitation Centre” for the following inpatient programs:
- Acquired Brain Injury (ABI) Service.
- Locomotor Care / Complex Orthopedics.
- Neuromuscular Care (including ALS, MS, Cerebral Palsy and spinal cord injuries).
Referrals should be sent via fax for the following inpatient programs

For patients being referred directly from the community
All patients must be referred by a physician or nurse practitioner.
A referral for outpatient Physiatry consultation should be sent using the Ocean eReferral system using the listing “The Ottawa Hospital Physiatry Consult – TOH Rehabilitation Centre” for the following services:
- Acquired Brain Injury.
- Amputee Rehabilitation.
- Musculoskeletal Rehabilitation.
- Neuromuscular Rehabilitation.
- Spasticity Management.
- Electromyography.

Once a referral is received
Each referral is reviewed by the rehabilitation team to determine if the program can help your specific needs so you can achieve your personal rehabilitation goals. The outcome of this review will be communicated directly to the physician or health care team member who submitted the referral.
If you are on the waiting list, you will be contacted as soon as a bed is available, and an admission date will be set. Specific instructions for preparation for your inpatient stay will be given at this time.
